Responsibilities:

Assessment and diagnosis of patients using other patient / family-centered approaches as needed, such as other health / care-related psychological concerns.

Develops need-based interventions and makes professional decisions in consultation with the supervising social worker to provide problem-based, culturally appropriate care.

When appropriate, the community participates in assessing the needs of human services and plays a leading role. Collaborate with community members and HR providers to develop approaches to meet the needs.

The overall purpose of this position is to help patients maintain, regain, or enhance their social work when their ability to work is affected by a real or potential illness, disability, or injury. The contents of these services range from prevention and education to helping patients cope with significant changes in social roles, future expectations, and life goals as well as changes in health status. Must have a Social Worker License with unlimited restrictions in any state.
